1966 Reggie Jackson Official Minor League Publicity Questionnaire

1966 minor league player questionnaire filled out by Reggie Jackson during his first season in professional baseball. Following a short two-year career at Arizona State University, Reggie Jackson was selected by the Kansas City Athletics with the second overall pick in the 1966 baseball draft. After his signing, Jackson was sent to play with the Lewiston Broncs of the Northwest League. Prior to joining his new club, Jackson, like all other minor league players at the time, was required to fill out an official questionnaire for use by the league's media services. Offered is the standard one-sheet player questionnaire filled out by Jackson in 1966. In addition to the usual information asked (home town, date of birth, high school, marital status, etc.), Jackson also responded to additional questions to provide information for media use. To the question of "List any interesting experiences while participating in school or college sports" he answered "First college football game before 40,000 people." The question of his "Ambition in Baseball" garnered the reply "To get to Big Leagues as quick as [possible]." He also made note that his "...most interesting or unusual experience in baseball" was "Hit HR out Phoenix, Stadium First college player to do so (est.) 480 ft." While the questionnaire is not signed, all of the written responses (including his full name of "Reginald Martinez Jackson") have been printed in blue ink personally by Jackson, with the text grading "9/10". The document, issued by William J. Weiss (the chief statistician and then head of publicity for seven different minor leagues), bears the red date-stamp of "Jun 18 1966" in the upper right corner. Measuring 8.5 x 14 inches, the questionnaire displays three horizontal folds and a few tiny tears along the top border. This is a fascinating and unique document from the dawn of the professional baseball career of future Hall of Famer Reggie Jackson. Excellent condition overall.
